Position Summary:

The Cardiovascular (CV) Navigator provides clinical oversight to the Open Heart Surgery program to ensure continuity of care and excellence in quality for the Coronary Artery Bypass Graft (CABG) patient population. The CV Navigator is responsible for ensuring that established best practices and protocols are followed throughout the continuum of care for this patient population, from patient identification and screening to 90-day post-op follow up care.

The CV Navigator will work with the CT Surgeons to coordinate the care of the CABG patient. All aspects of utilizing evidenced based care and best practice will be overseen by this position. The CV Navigator will facilitate the development and adherence of agreed upon standards of care for selection of CABG patients for surgery, pre-operative tests and screenings, post-operative nursing care in the critical care unit and progressive care unit, discharge planning, and follow-up care. This position will assist with the development of initiatives based upon review of Society of Thoracic Surgeons (STS) data to improve outcomes at St. Josephs/Candler Hospital.

Education:

BSN Required, Masters Nursing Preferred (NP or CNS)

Experience:

3 Years of Cardiovascular ICU Required
