is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay.  The J.C. J. C. Lewis Primary Health Care Center is seeking to add a Community Health Coordinator to our team. The Community Health Coordinator is responsible for engaging and building trusting relationships with adults who are at high risk for experiencing health issues surrounding homelessness. This role will identify, assess, and refer homeless adults within the clinic and multiple outreach settings to care and services. As part of an interdisciplinary team, this role will identify barriers and help clients access a variety of resources including specialty care services and housing. The Community Health Coordinator will also conduct individual health education and risk reduction interventions.

Qualifications:

  • Bachelors Degree in Social Work, Human Services, or other helping profession
  • Three years’ experience in social work, human services, clinical setting experience or a combination of education and experience
  • Minimum of three years of experience reaching out to vulnerable individuals who are often the most difficult to serve
  • Minimum of three years of experience  in connecting individuals to essential services including healthcare, mental health, and addiction services
  • Must possess excellent computer, communication and customer service skills
  • Must be skilled in crisis management and documentation and have experience working with clients in crisis
  • Previous knowledge of substance abuse, mental health and bilingual ability a plus

Primary Responsibilities:

  • Through service provision and regular supports for engagement, build low-demand relationships with assigned clients that results in improved engagement with services.
  • Identify and engage homeless individuals on the streets, in shelters, meal programs, hospitals, and other sites and serve as a bridge to connect clients with services at Health Care for the Homeless.
  • Using comprehensive knowledge of common health-related issues including HIV disease, mental health and substance abuse, effectively assess homeless individuals for high-risk behavior and refer to appropriate services
  • Effectively conduct health education and risk reduction interventions in outreach settings
  • Actively participate on an integrated, multidisciplinary team to develop and implement an integrated treatment plan, within the context of an individual or family’s unique social needs, as well as contribute to interdisciplinary case conferences on an ongoing basis.
  • Serve as a bridge to connect clients with services at J C Lewis Primary Health Care Center and services throughout Savannah/Chatham County, through independently connecting with multiple organizations and services to understand their capacity to support these individuals.
  • Navigate clients through the housing process which may include Savannah Chatham County Authority for the Homeless. Chatham County Housing Authority, community based housing programs, and other housing resources
  • Actively work as part of an interdisciplinary team in tracking clients disengaged from medical follow-up
