The J.C. Lewis Primary Health Care Center is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay.  The J.C. The J.C. Lewis Primary Health Care Center is seeking to add a Dietician. The Dietitian functions as an active member of the integrated clinical team. The Dietitian provides individual and group nutrition counseling and education to enhance the overall health of the patients.

Primary Responsibilities:

• Assess and analyze dietary practices and body composition of patients in the context of health
• Counsel patients on achieving and maintaining a level of body mass, body fat and muscle mass that is consistent with good health
• Provide personalized meal and snack plans to promote achieving short- and long-term goals for good health and disease management
• Education patients in food selection and preparation
• Collaborate with the Respite Case Manager and kitchen staff to develop appropriate menus for the Respite patients
• Develop and deliver nutrition education presentations, demonstrations and/or events on various topics related to nutrition for good health and/or disease management
