LPN Hospice House-Day Shift

SUMMARY: The LPN uses experience, knowledge, judgment and skills to provide nursing care to patients and families. The LPN is committed to the philosophy, goals and objectives of the hospice concept. The LPN works under the direct supervision of the RN while consistently demonstrating the Hospice Savannah values and Service Excellence standards.

QUALIFICATIONS: Graduate of an accredited school of Nursing, with a current GA LPN License. Minimum of one year of experience in hospice, oncology, med-surg or home health providing direct patient care. Current CPR certification, driver’s license, automobile insurance and reliable transportation. Ability to successfully interact with patients, families, physicians and the community. Excellent assessment and documentation skills.

PHYSICAL DEMANDS: Position requires moderate visual concentration for patient charting/documentation. Position carries a risk of exposure to infectious diseases requiring standard safety precautions. The LPN requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 lbs. utilizing proper body mechanics.

WORK ENVIRONMENT: This position works 24-36 hours per week, 12-hour shifts, 7a-7p, weekends and holidays as scheduled, with occasional requests to work additional days/hours as needed. At times, due to patient census and acuity, this position may be required to remain on site during meal breaks. This position encounters stressful situations with patients and their families regarding end of life care.
