J.C. Lewis Primary Health Care Centeris a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. Position Summary:

The Licensed Clinical Social Worker (LCSW) functions as an active member of the interdisciplinary clinical team.The Licensed Clinical Social Worker (LCSW) provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs within a primary health care environment.

Qualifications:

  • Bachelors or Masters degree (MSW) in social work
  • Must be Licensed Clinical Social Worker by the GA Composite Board for Social Workers.
  • Ability to interpret the social, emotional, and behavioral problems as related to the medical condition and develop an appropriate treatment plan.
  • Demonstrated competence in reacting to and handling emergencies.
  • Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline.
  • Strong charting/documentation skills.
  • Ability to effectively communicate with people at all levels and from various backgrounds.
  • Must be able to speak, read, write, and understand the primary language(s) used in the workplace.
  • Good judgement with the ability to make timely and sound decisions
  • Strong organizational, problem-solving, and analytical skills
  • Ability to manage priorities and workflow
  • Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.

Primary Responsibilities:

  • Diagnose and treat acute, episodic behavioral illnesses
  • Deliver psychotherapy treatments
  • Perform comprehensive behavioral examinations of patients
  • Develop and implement patient management policies and procedures
  • Assist in resolving psychosocial problems which may interfere with medical treatment or rehabilitation of the patient/family.
  • Advises, counsels, and when appropriate instructs family in patients social needs.
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with collaborating psychiatrist, physician or specialist regarding patient care
  • Assists the Performance Improvement initiative by developing and implementing Performance Improvement indicators with appropriate follow up monitoring.

Other Duties and Responsibilities

  • Maintain licensure, to include the procurement of appropriate CMEs
  • Participate in agency meetings, in-service trainings and workshops as deemed appropriate
  • All other duties as assigned
