Benton House of Bluffton, an assisted living and memory care facility, has an opening for a LPN.  The Nurse (LPN) has the direct responsibility of assisting the Resident Services Director with the direct supervision of care services within the community. The LPN is required to function independent of direct supervision, being attentive to the resident’s care and the community support services.

MAJOR TASKS, DUTIES AND RESPONSIBILITIES

Assists the Resident Services Director in the care oversight of all residents, including thedissemination of information to families and staff.

Complete care plan assessments as directed by the Resident Services Director.

Management of the resident medication oversight and assistance, including all documentation.

Upon new admissions, completing all required documentation including nursing assessments,care plans, and other assigned forms as requested by Resident Services Director.

Participate in community functions, including family socials and open houses.

If unable to solve problems identified by family members, the problem is referred to the Resident Services Director.

Supervise the maintenance of resident charts, review charting performed by other staff as appropriate.

Assure in-house ancillary medical services, such as podiatrist, doctor visits, dental visits,psychiatric, lab, e-ray, ambulance, etc. are scheduled and followed completed.

Facilitate continuity of care for those residents receiving home health care services.

Communicate with the Resident Services Director and assigned physician regarding change incondition of the resident, followed by proper documentation.

Be in compliance with the State of South Carolina licensing criteria at all times.

Perform ongoing assessments of resident physical and psycho-social needs and coordinate withother departments to assure quality, pro-active care as requested by Resident Services Director.

Other duties as assigned by the Resident Services Director.

Perform inventory of all medications and reorder when needed, while maintaining thedocumentation of said actions.

Adhere to all policies and procedures of the community.

To act at all times in a professional manner to visitors, staff and residents in the community.

Attendance on the job as scheduled.

Must be a minimum of eighteen years old.

QUALIFICATIONS

Must hold a current LPN license in the State of South Carolina.

Successful completion of drug screening.

Must obtain a satisfactory criminal record check per State guidelines.

Must have received a tuberculosis screening and physical examination within twelve monthsprior to employment (or as State Guidelines Require) to assure that the employee is free ofdiseases communicable within the scope of employment and physically qualified to work.

Ability to communicate effectively with families, care assistants and management.

Must be able to work under stress and in emergency situations.

PHYSICAL DEMANDS

Must be able to work under conditions which require sitting, standing, walking, reaching,pulling, pushing, and grasping with risk of environmental exposure to communicable disease.

Must be able to talk and hear effectively in order to convey instructions and information topatients and staff members.

Must be able to communicate effectively and sensitively with other staff members, referralsources, patients and their families and physicians.

Must be able to lift 50 pounds.

Requirements for personal protective equipment include infection control kit and rubber glovesand smock and non-slip shoes.

OCCUPATION EXPOSURE/PERSONAL PROTECTIVE EQUIPMENT

Supervisors work in all areas of the residence.

Supervisors are subject to reactions from dust, disinfectants and cleaning chemicals and may beexposed to infectious waste, diseases and other conditions.

Supervisors are expected to use personal protective equipment and supplies to prevent burns,falls, and infection and follow safety policies and procedure.
